Client and Policies Forms

First time clients are required to fill out and sign a Client Health History, Client Intake, Policies & Privacy Policy Notice Form before their initial massage is performed. These forms will be updated periodically and as necessary. Attached below for your convenience are the required forms for you to print, fill out and sign prior to your massage session. Taking this step can save time in the client interview process.
